On 19 February 2021, the inquest into the death of Janet Scott concluded at the Nottingham Coroner’s Court. William Irwin represented the National Probation Service (NPS), part of the Ministry of Justice.
Mrs Scott was murdered by her former partner, Simon Mellors. Mellors had previously murdered another partner, Pearl Black, and was sentenced to life imprisonment for her murder in 1999. He was released on conditional licence in 2014. At the time he killed Mrs Scott he was subject to supervision by an offender manager (i.e. probation officer), Andrew Victor.
The Assistant Coroner for Nottinghamshire – Jonathan Straw – concluded that Mrs Scott had been unlawfully killed.
He found that Mr Victor (who was separately represented) had made errors in Mellors’s management which were causative of Mrs Scott’s death.
He also found that the NPS office in which Mr Victor worked had had a high workload since the national Transforming Rehabilitation reforms in 2014. However, the Assistant Coroner held that the high workload was not causative of Mrs Scott’s death.
The Assistant Coroner was satisfied that he was not obliged to make a report to prevent future deaths under Regulation 28 of the Coroners (Investigations) Regulations 2013.
