Ellen acted for an NHS Wales Health Board in an inquest into the death of a teenager who was under the care of outpatient mental health services and social services at the time of her death.
The inquest involved evidence from the deceased’s boyfriend, mental health services, social services and her school, and examined issues arising out of boundary issues with different local authorities.
After hearing evidence and submissions, the Coroner reached a conclusion of suicide, accepting that there were no causative failings or issues requiring a Regulation 28 report.
