Inquest into the death of Joshua Delaney

04/02/2026

Saoirse was instructed by the Family at a 10 day jury inquest at Inner London Coroner’s Court regarding the death of an 19 year old man who died of a Propranolol overdose. The Inquest raised a number of complex Article 2 ECHR issues concerning operational police failures: failure to communicate “self-harm” markers on Joshua’s PNC to attending police officers and to act appropriately when attending his home just an hour before he died. Saoirse made persuasive submissions regarding the prevention of future deaths to ensure GPs appropriately prescribe and monitor Propranolol prescriptions given the number of deaths caused by overdoses.
