Leicester City King Power Stadium Helicopter Crash Inquest (2025)
The inquests into the deaths of Leicester City Football Club owner Vichai Srivaddhanaprabha, along with two of his staff, Nursara Suknamai and Kaveporn Punpare, pilots Eric Swaffer and Izabela Roza Lechowicz. The helicopter crashed shortly after take-off from the Stadium. A duplex bearing associated with the tail rotor seized depriving the pilot of the ability to control the aircraft. The jury returned a conclusion of Accident and a narrative.

Inquest into the death of Wayne Brown (2025)
Keith represented the West Midlands Fire service at the inquest into the death of Wayne Brown, the UK’s first black chief fire officer who took his own life following allegations of harassment and allegations related to his qualifications
Inquest into the death of Lyla Windell (2025)
Lyla tragically died after choking on food at a nursery, despite the best efforts of the staff. Keith represented the nursery
Inquests into Three Drownings in Cornwall During COVID lockdowns
Three men drowned in separate incidents in the sea at Treyarnon Bay, Porthcurno, and Church Cove in Cornwall. The deaths occurred during the Covid lockdowns in 2020, when RNLI lifeguard services were suspended due to pandemic restrictions. The inquests considered whether in the absence of any common law duty on landowners to provide lifeguards on beaches the coroner should issue a prevention of future death report. The coroner was persuaded that would not be appropriate in circumstances where both domestic and European law are clear there is no such duty. Keith represented The National Trust who owned (part of) the beaches concerned.
Inquest in to the death of Jacob Billington (2024)
Jacob Billington was fatally stabbed during a series of unprovoked attacks in Birmingham city centre. The perpetrator, diagnosed with paranoid schizophrenia, had been released from prison months earlier without mental health support. Keith represented a Health Board that provided mental health in reach services at a prison housing the perpetrator immediately prior to his release just as the Covid lockdown came into force.
Inquest in to the Death of Oliver Steeper (2024)
Keith represented the nursery in this tragic case in which a nine-month-old child tragically died after choking on food. The coroner concluded that there was “no evidence which would show failures by nursery or its staff”.
Inquest into the death of Jean Langan (2024)
Jean Langan died as she made her way across the car park at Derriford Hospital when she was struck by the downwash of an incoming helicopter landing at the hospital’s helipad. Keith was instructed by the Civil Aviation Authority
Inquest into the Death of John Coles (2023)
Keith represented Heathrow Airport Limited in this fatal airside road traffic collision. The case involved difficult factual and expert evidence concerning visibility and visual interference airside at the country’s major airport.
Inquest into the death of Lauren Bridges (2023)
Lauren Bridges, who was from Dorset, was an “out of area” patent detained under Section 3 of the Mental Health Act in the Psychiatric Intensive Care Unit at the Priory Hospital Cheadle Royal, near Manchester, which was 260 miles from her home. Lauren, then aged 20, was found unresponsive on 24 February 2022 and died on 26 February 2022 at Wythenshawe Hospital. The jury found that she did not intend to end her life and concluded her death was misadventure.
Inquest into the death of Dr Nicholas Harrison (2023)
Dr Harrison was killed by his son, who was suffering from mental illness. His family sought medical attention for him over several months. He was detained under the Mental Health Act but absconded from the ward. He then made his way to his parents’ home where he killed his father. The inquest raised complex factual, expert, and procedural issues. Keith acted for Swansea Bay University Health Board.
Shoreham Airshow Inquests (2022)
Instructed by the Civil Aviation Authority in the Inquests in the deaths of 11 people on 22 August 2015, when a Hawker Hunter participating in the Shoreham Air Show crashed onto the A27. The Senior Coroner concluded that the deceased were unlawfully killed.
Celia Marsh Inquest (2022)
Keith represented Planet Coconut in the Inquest into the death of Celia Marsh who suffered an allergic reaction to a wrap purchased from Pret a Manger.
Emiliano Sala Inquest (2022)
Keith represented the Civil Aviation Authority in the Inquest into the death of Emiliano Sala, the professional footballer, who died when the aircraft he was flown in crashed en route from Nantes to Cardiff.
Croydon Tram Inquests (2021)
Represented Transport for London at the inquests into the deaths of seven people killed in the Croydon Tram crash at Sandilands on 9 November 2016.
Vauxhall Helicopter Crash Inquest
Keith acted for the Civil Aviation Authority at the Inquest into this high profile case in which a helicopter collided with a crane on top of St Georges Wharf Tower, then under construction in Vauxhall.
